High performance primary care contributing to the quadruple objective in heatlhcare systems
Abstract
Primary Care is both base and center of successful healthcare systems. As the ideal first contact for patients, accessibility is an essential attribute yet most primary care struggles to meet patient needs and inquiries in a timely manner. Team based care provides some relief from the inevitable access bottleneck experienced by primary care providers. Access without continuity leads to fragmented care and higher costs through duplication and unnecessary testing. Continuity of teams is a successful strategy to guide and reassure patients when they require a care interaction and can decrease inappropriate care by Emergency Departments. New technologies are facilitating these new relationships and moving care from face to face to more virtual or asynchronous interactions. Relationship remains the cornerstone of primary care. Designing systems within primary care which enhance access, relationship and continuity promote higher workplace satisfaction for all participants.
