INTEGRAL HEALTH CARE PROJECT DIABETES MELLITUS AND ARTERIAL HYPERTENSION IN THE MUNICIPALITY OF UBERABA
Abstract
The Municipal Diabetes and Hypertension Center of the Municipality of Uberaba (CEMDHI) aims the qualification of health care for diabetic, hypertensive and obese patients registered in the Primary Health Care (PHC) network of the Municipality of Uberaba. Has as proposal the comprehensive care of patients based on the empowerment of Primary Health Care teams and better treatment conditions for users with these morbidities. For this purpose, it was elaborated a permanent education program about these areas for the primary health care assistance teams. It was also implemented a matriciament process comprehensive and agile of cases. Are being used the resources of Telehealth and weekly in person meetings. Digital applications are being developed to connect patients, PHC and CEMDHI team. The Municipal Center has diabetes services for adults and children, gestational diabetes, diabetic foot diabetes, obesity and hypertension. A Flowchart for referencing of patients to SAMDHI and of return of these patients to PHC it was elaborated.
